Step 1: Determine DDA Eligibility

An individual must apply to the DDA to determine eligibility. A developmental disability is generally defined as a severe chronic disability (not solely a mental illness) that arises before age 22 and requires significant support for independent living.

Step 2: Complete and Submit the Application

The application for eligibility is available through the DDA website or by contacting the Regional Office.

Step 3: Comprehensive Assessment and Coordination of Community Services (CCS)

After applying, a DDA representative will typically contact you to schedule an interview and comprehensive assessment. A Coordinator of Community Services (CCS) will be assigned to assist you with the application process and, if eligible, help you with Person-Centered Planning.

Step 4: Eligibility and Service Prioritization

Once eligibility is determined, your need for services is prioritized. Priority categories include:

• • Crisis Resolution: For those at immediate risk (e.g., homelessness, abuse/neglect, or caregiver health failure).
  • Crisis Prevention: For those at substantial risk of a crisis within one year, or whose primary caregiver is 65 years or older.
  • Current Request: For individuals who need services within the next three years but are not in a crisis
  • Transitioning Youth (GTYI): Students transitioning from high school to adult services (typically age 21–22).

Supports Only: Individuals eligible for DDA services who only require less intensive supports.

Step 5: Person-Centered Planning and Waivers

If funding is available, the CCS will help develop a Person-Centered Plan based on the individual’s goals and needs. DDA services are primarily provided through the Community Pathways Medicaid Waiver.
